The duly filled form to be submitted along with the following documents; 1. Passport photos – 4 nos. 2. Passport photos (3 each) of both parents and/or authorized pick up person(s) 3. Copy of the child’s birth certificate 4. Copy of the child’s immunization history/card Write up about the child To fill the Child’s Personal Record. Fee Payment.
